On call
Long nights has never been so fun!
Yesterday was a hell of a day = )
Lets see...
The day started off late - i woke up at like 915am and leisurely got to hosp by 10am, since i was staying late last night. The morning went slowly, afternoon too - the only excitement was that i went to take blood. Which was a success.
Anyhows, mid-afternoon i was complaining to lina abt how i want to watch other procedures, and she hooked me up with Chin, this new advanced cardiology trainee, who introduced me to all da other pple and i went in to watch pacemaker insertion! This guy steve explained all the pacemaker types and functions to me, and then i went in to watch the operation. I didn't do much, but it was quite fun!! but now at least i'm setup to go watch other stuff like angio, since the interventional reg knows who i am now! So yea...After the operation I followed chin ard to the xray room, and while there we chatted about how to become a cardiologist. Apparently, you have to do 2 yrs of basic intern postgrad training, then become a medreg, then sit the written exam, then pass the clinical exam, then you have to get accepted into the advanced training scheme. That's the hard part. Then you train for 3 years, and you have a logbook of things to do e.g. 600echos etc, then you can become a fellow and consultant!

Getting accepted into the scheme is hard. It is probably the same as getting accepted into the advanced training scheme for surgery - except i guess there isn't the 4 years live-or-death thing that happens. But then again there may be the case where if the cardiologists don't like you, they'll never accept you kinda thing (like in ortho) --- so....
In conclusion its a hard road ahead even if i wana go along the pathway of a medic!

So then after that i paged lina and found her. I was supposed to be on call with her for that night you see - residents do long days till 1130 here, shocking eh. So I found her, we went to this ward to review a pneumonia patient with chest pain. After that went to this ward to take blood from an grumpy patient. I failed, so lina tried, and she failed. Then after that i went to take another blood from another patient, from the line tho so i didn't have to poke her. Funnily enuff i failed that as well...haha - but lina got it. (she was being tested for cushings).
And then - we went to the dialysis ward, where there's pple sitting ard waiting for the machines to finish working. There's this old lady with a tube sticking into her subclavian vein, and the resident let me stitch the tube into the skin on her chest!!! While she jus watches and directs me!!
So we got there, got setup, this tall young guy (who's a nurse but i thot was a reg), who was REALLY patient, stood beside me and directed me on what to do and what to hold and stuff. I tell you, this was like a minioperation man (to me...haha) First i had to scrub up, then wear the surgical gloves, then set out the equipment in the sterile way, then clean the tube area up. Then I injected local anaesthetics (MY FIRST TIME GIVING LOCAL!!! AND I GAVE IT TWICE!!! OHHHH YEA), then i waited, and i took the stitch with the needle holder, took a bit of skin and tied it around the little hole on the plastic around the tube sticking into the subclavian (the vein under the clavicles). Then i tied a knot, cut it off, and did the same to the other side. Exciting~! haha it mite sound quite simple and straightforward to you, but:
1. I have to try to be sterile, meaning the sterile equipment and my hands are not allowed to touch any other stuff which is not sterile.
2. These was my 2nd and 3rd real stitches on a real life person
3. The first time i was the only one doing the procedure
I was so happy!!! haha lina and the nurse were so patient with me, watching and directing me on what to do and stuff even tho i was confused and i even confused myself on how to tie a knot at the end. But it was cool.
After that we went to another ward and i had to put a line into the old woman. Her veins are not that nice, i tried once but a big lump formed under her arm, and the saline wouldn't flush thru so i failed. Then i found another vein, and i stick it in, and the needle fell out, and while i was about to try another time, lina came along, saw this beautiful fat vein, and i stick it in, saw the flashback, and it worked!! woohoo!!! success.
And then there was another man who needed a line. So lina took me there, looked at his veins, and left me doing it while she went to take blood off grumpy man. I foudn the vein, and it all went smoothly!! I didn't even make a mess of blood on the bed like i usually do!! THe saline flushed thru the vein beautifully!! The cannula was a bit horizontal i guess but it worked ,and thats all it matterS!! heheh
So after that we saw this dialysis patient with enterococci uti with this MEGA ASS fistula on his left hand, it was like 2.5cm in radius wide vein?! holy shit, it was like a snake under the skin!! mega. We admitted her, i examined her in front of lina.
AFter which i went home.
Wow!! I duno, it mite jus sound like nothing to you-s , but to me, that was like one of the mosty fun night i had!!! one excitement after the other! I was sooo happy!! I wish i can do more of this - gona hang ard lina more nxt time she goes on call!! SHe's agreed also so yup!! me happy...!!!
Gotta catch da bus soon, i've been typing for 30min.

Today I went to clinic and heard this mitral valve prolapse click and this soft end systolic murmur of mitral regurgitation which is only elicited when he rose from a squatting position.
COOL!
IT was like straight out of the tape...sounded like. Wow.
Anyhows i really gtg now b4 i missed the bus, the nxt one is at 8pm!!! >< i havn't had dinner yet!!!
This is what happens when i dun have internet for 2 days...i get verbal diarrhoea..
I mite go to syd for the whoel weekend 2mr.... ithink i will.....heheh go fri and come bk sun!! yay...
